Snorting formula out the nose doesn't necessarily mean that aspiration pneumonia is going to develop. The very first signal of a problem is a sudden drop in appetite, or that plus unusual lethargy. Once a baby that was happy to eat suddenly isn't, it is time to start dosing. You don't want to wait for the "clicking" but you also don't want to give antibiotics if they aren't needed.
