    Default what AB for lactating mom??

    thank you if you know or can help me research.. I am having brain fade.
    I need to know what I can use for a lactating/nursing mom... and a pain aid as well..
    metacam? prednisone? tramadol? ibuprofen?

    ABs,
    amoxy, keflex, metronidazole, cipro, doxy, SMZ-TMP.
    Banana is in my bathroom.
    babies look good, a boy and a girl... seem sleepy.. fat, one eye out of four is open...4.5 weeks old.
    Didn't see her out back for a couple days, started looking, found her in her nestbox not looking good...yes alert, but an obvious 'front'.
    Got her in a cage, but not before she shot out of box with obvious signs of injury...she allowed me to handle her.. very willingly...
    very concerned... will let her hunker in for a couple hours and will start investigating and triage.. had babies seperated, but thought she may feel better with them..
    if she declines or snubs them, I will start FV and seperate...
    ... our big matriarch is down

    JIT I did some quick reading.
    Pain killers...looks like ibuprofen is okay
    Abs...not "our" first choice, but Amoxicillin appears to be okay as does Penicillin.

    Cipro is recommended against due to amounts found in breastmilk.

    Keflex appears to be okay too....actually better than the 'cillins, as it has not been traced to breastmilk even in small amounts.

    What MM said! I just found the same thing. It was for humans, but looks like if Amoxi will work not much ends up in breastmilk.

    Jackie - I just found this about Tramadol.

    Summary of Use during Lactation

    The excretion of tramadol into milk is low and even lower amounts of the active metabolite, O-desmethyltramadol, are excreted. With usual maternal dosage, the amount excreted into breastmilk is much less than the dose given to newborn infants for analgesia. A study in breastfed newborn infants found no adverse effects attributable to tramadol. Tramadol is unlikely to adversely affect nursing infant and is acceptable to use during breastfeeding. However, monitor infants for increased sleepiness (more than usual), difficulty breastfeeding, breathing difficulties or limpness, and contact a physician immediately if any of these occur.

    Looks like Metro is a big No No.

    Metronidazole Breastfeeding Warnings

    Metronidazole is secreted in breast milk in concentrations similar to those found in plasma. Because of the potential for tumorigenicity, shown for metronidazole in mouse and rat studies, decide whether to discontinue the drug, taking into account the importance of the drug to the mother.


    Doxy appears to be ONLY when the benefits to the mother outweigh the risks to the babies.

    Prednisone Breastfeeding Warnings

    In one study of six lactating women, prednisolone milk concentrations were 5% to 25% of corresponding serum concentrations. The authors estimated that a nursing infant would be exposed to negligible amounts of drug with maternal doses of 20 to 40 mg per day. The excretion of prednisolone into breast milk has been evaluated in six women on chronic prednisolone therapy. Milk prednisolone concentrations ranged from less than 10 to 106 ng/mL in patients receiving 10 mg to 45 mg per day (N=5). In a patient receiving 80 mg per day, milk prednisolone concentrations ranged from less than 10 ng/mL prior to the morning dose to 317 ng/mL at one hour post-dose. The peak milk to plasma concentration ratio ranged from 0.12 to 0.25. The authors recommend that nursing be withheld for four hours after administration of doses greater than 20 mg. In a recent study, prednisolone excretion into breast milk was evaluated in three women following administration of prednisolone phosphate 50 mg intravenously one time. During the six-hour study period, 0.010% to 0.049% of the administered dose of prednisolone was recovered in breast milk. Nursing infants would be expected to experience minimal exposure. Data from this study also suggest that exchange of unbound prednisolone between breast milk and serum is rapid and bi-directional.

    in the middle of chopping veggies.. knock knockk
    i just took in cat caught baby... finders/cat owner said cat was chewng on baby.. so precious..sadly seizing...

    i requested a belled collar..i knew finder from her bringing me nigel
    but he is here... praying he can hang on...will hold him one work one handed..poor little
    have not cked on big b yet....

    thanks...mind is racing to make decisions...

    From everything I was able to find it appears that ibuprophen is okay while pregnant and/or lactating. The amounts that are transferred into the breastmilk are minimal and equate to less than 1/8 of an infant dose.

    Although, everything I read did say that when determining whether to give any med to a pregnant or lactating mom the benefits to the Mom should outweigh the risks to the babies.

    Since we regularly administer ibuprophen to babies, I would tend to think that it would be fine for the babies to ingest a little bit through the breast milk. If she is in that much pain due to her injuries, she may very well reject all of the babies to save herself. A bit of pain killer may make it all a little more bearable for her. I think in this instance the benefit to Mom outweighs the risk to babies.

    A very wise rehabber once told me that I could give Tramadol and Motrin to an injured fuzzer.... 1 hour apart from each other.

    If her injuries are bad enough, it looks like Tramadol and Motrin are both on the OK list.

    okay...that's where we'll start... ibuprofen...
    probably should just pull her babies,,,
    okay, have cc baby on heat, and 2 ccs FV, smz-tmp until I can get some pred made and cipro...

    banana..will go ck on her andf make the call in 20 minutes..
    insiders wityh teeth are feed
    outsiders in captivity lunches are on the tray...
    flyer tray almost done... then will make meds and admin to baby..
    start to get RRRB and Twitch ready for their moms arrival...
    .. Oh yeah, I don't know any Twitches...
    sorry KB... only RRRB is here
    sat here all night with Djar and then on the phone the rest of the night with SSM..
    woke to no coffee in the house... sent Fred who has no clue...
    but have enough caffine now to chalange any red bulll....
    bring it on!!!
    thanks will let you know what happens..
    prep cook, maid, nurse, nurse practioneer, pharmacist...
    and still need to chop wood before the rain get here... freds hanging crown across town...
    I was so enjoying the calm.

    oh yeah, took in a one eyed flyer last night... it's sq time again.
