NIles, Il. 5-6wk grey with teeth broken and possible other injuries NEED HELP PLEASE

Was contacted by someone in Niles, Il. about a baby squirrel and am hoping there is someone near there that that can be of help. Fall injury, One bottom and both top teeth broken, raspy breathing, small bleeding from one eye, strong but resting quiet now. Needs some medical attention from a rehabber or vet most likely. Anyone near the area, not sure where in IL. it is. Trying to find out if I can post the finders email here now.
