Help for Karma who is not peeing

I spoke to Karma's mom today and she said Karma was not peeing when stimulated as she usually does. Karma is an adult who is partially paralyzed in her back end. We decided that she may also be in her first heat. I got a call this evening and she is not peeing and her privates seem swollen. She said it almost seems they are swollen closed and not allowing her to pee. Anyone have any ideas about what she should do? She does not have vet access.

The only thing I can think to suggest outside cather
is to med to reduce swelling and maybe make it more
comfortable for her to pee.
Wondering if she has tried a soak?

I would start her on bactrim and metacam, to try to bring swelling down, but also to keep bacteria at bay, from not peeing. Can she express it manually?
